29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section: On or about 22 November 2016, the employer's lockout/tagout program did not include documented procedures that clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of potentially hazardous energy. Employees gaining access to all production equipment, to include the Amada Platesaws and Sunstrand Mill to perform servicing, unjamming and maintenance were exposed to caught-in hazards. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1910.147(c)(7)(i)(B): Each affected employee shall be instructed in the purpose and use of the energy control procedure. On or about 22 November 2016. the employer failed to provide training to each affected employee who was exposed to Lock out/ Tag out operations. The affected employees were not familiar with the purpose, use, procedures, and prohibitions relating to potential energy sources or the dangers of stored energy. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc. On or about 22 November 2016, machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by the following machines: a. Sunstrand Mill: The machine has moving parts that were not guarded to prevent employees from crushing hazards when the hydraulic rams extend to hold the metal in place for the milling process. There is no guarding in place to prevent the employees from gaining access to the milling operations and the rotating cutting head. This exposes employees to struck by, crushing, caught in hazards. b. Amada VM-1200 Vertical band saw: The blade guard was not adjusted to protect the unused portion of the blade from accidental contact. Employees were exposed to lacerations and/or amputations from inadvertent contact. c. Amada VM-2500 Vertical band saw: The blade guard was not adjusted to protect the unused portion of the blade from accidental contact. Employees were exposed to lacerations and/or amputations from inadvertent contact. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
