29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch. a) Machine Shop, 20th and Market Streets, Ashland, PA - The Cincinnati Electrical Tool Company BEA Model 102 abrasive wheel grinder lacked adjustable tongue guards on the left and right abrasive wheels exposing employees to struck-by hazards, on or about November 21, 2016.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment. a) Waste paper baler, North 20th Street & Market Street, Ashland, PA - Employee access to the lockout-tagout point and electrical panel for the REC baler was blocked by more than fourteen (>14) hoppers filled with waste paper exposing employees to slips, trips and falls while trying to access the equipment, as observed on or about November 21, 2016.
