29 CFR 1910.101(b): Compressed gas cylinder storage areas were not prominently posted with the names of the gases to be stored.  Storage of compressed gas cylinders were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6.  a) At the Plymouth/Black Mountain Hotshot facility, oxygen gas cylinders in storage used for welding did not have their regulators disconnected and valve protection caps installed.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour.  a) At the Plymouth/Black Mountain Hotshot facility, oxygen and acetylene gas cylinders stored together under the stairways for more than a month were not separated a minimum distance of 20 feet.

29 CFR 1910.157(e)(2): Portable fire extinguishers were not visually inspected at least monthly.  a) At the Plymouth/Black Mountain Hotshot facility, portable fire extinguishers were not inspected every month, the last inspection dates were March 10, 2016.  The USDA Forest Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.157 (e)(2), which was contained in OSHA inspection number 957389, citation number 2, item number 2 and was affirmed as a final order on 04/01/2014, with respect to a workplace located at Descanso Ranger Station 3348 Alpine Blvd., Alpine, CA 91901.

The USDA Forest Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.334(a)(2)(ii), which was contained in OSHA inspection number 911911, citation number 1, item number 11 and was affirmed as a final order on 08/08/2013, with respect to a workplace located at USDA Forest Svc., 41374 North Shore Dr., Big Bear Station, Fawnskin, CA 92333.

The USDA Forest Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.1200(e)(1), which was contained in OSHA inspection number 642758 citation number 1, item number 1 and was affirmed as a final order on 04/02/2013, with respect to a workplace located at USDA Forest Svc., 701 N.
