29 CFR 1926.1051(a): A stairway or ladder was not provided at all personnel points of access where there is a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist is provided: a)Quiet Knoll Rd., Stockbridge, MA: On or about October 31, 2016, employees were climbing through an opening that was 31 inches above the ground, and there was no stairway, ladder, ramp, etc. provided for them to span that break in elevation while entering or exiting the structure they were constructing.
