29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilevered end:        (a) Jobsite, 127 Spencer Avenue, Pittsburgh, PA 15227: On or before November 17, 2016, an employee was working unprotected and exposed to fall hazards of approximately 16 feet to ground level in that scaffold stage extended beyond its left and right supports by approximately 36 inches.
