29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilivered end:  (a) Jobsite, 130 Kirk Avenue, Pittsburgh, PA 15227: On or before November 17, 2016, an employee was working unprotected and exposed to fall hazards of approximately 22 feet to ground level in that the scaffold stage extended beyond its left and right supports by approximately 5 feet.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:   (a) Jobsite, 130 Kirk Avenue, Pittsburgh, PA 15227: On or before November 17, 2016, an employee was working unprotected and exposed to fall hazards of approximately 22 feet to ground level while installing siding from a pump jack scaffold.
