29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section.    a) Jobsite: Dietz & Watson plant, Ground Meat Blending Room, location BR2, 5000 lb. Mixer-Blender - Employees conducting commercial cleaning activities were exposed to amputation hazards from the agitator paddles as the employer did not require the use of energy control procedures to control the potentially hazardous energy of the agitator during the entire cleaning process.  An employee cleaning the agitator through the discharge door suffered an amputation after coming in contact with the rotating agitator paddles losing three fingers.    Noted on or about 12/8/16.
