29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)    On or about November 21, 2016 at the construction site, employees were exposed to a fall hazards while attaching decorative bricks to an exterior wall without the working level being fully planked.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  .    On or about November 21, 2016, employees were exposed to fall hazards while climbing the frames of the scaffold to access the working level more than 2 feet above a point of access.

29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.     On or about November 21, 2016 on a window awning, employees were exposed to a fall hazard of more than 6 feet while attaching decorative brick without a fall protection system.
