29 CFR 1910.178(p)(1): Powered industrial truck(s) with defect(s) or in any way unsafe were not withdrawn from service until they were restored to safe operating condition(s): (a) Warehouse: Employees were permitted to operate defective fork lifts without having corrected the defective conditions. Defects observed on the trucks included hydraulic oil leaks, non-functioning signal and reverse lights, missing slip resistant covers from brake and gas pedals and damaged tires with missing rubber chunks.
