29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedures at least annually to ensure that the procedures and requirements of this standard were being followed:    a) Aspen Skiing Company at Aspen Mountain, 601 East Dean Street, Aspen, CO 81611:  On and preceding 11/18/2016, the employer did not conduct an inspection of the energy control procedures at least annually to ensure that the procedures and requirements of this standard were being followed.  The employer established a procedure for maintenance on lift towers to allow for immediate repair.  This procedure included pressing the e-stop, and then confirming the de-energization with the tower control, prior to working on the lift.  The employer did not conduct inspections of this procedure to verify consistent use of the e-stop and the communication with tower control.  Several employees were performing maintenance on a lift when the lift started up (a near-miss).  This condition exposed employees to amputation and fall hazards.

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, was not guarded:    a) Aspen Skiing Company at Aspen Mountain, 601 East Dean Street, Aspen, CO 81611:  On and preceding 11/18/2016, the employer did not ensure that the point of operation of an hydraulic press located in the maintenance shop was guarded.  This 10,000 psi hydraulic press was used to remove and install rings on single-piece sheaves.  The press was operated with a foot pedal adjacent to the point of operation, and no physical barriers, two-handed controls, interlocks, or other means of guarding was in place.  This condition exposed employees to amputation hazards.    b) Aspen Skiing Company at Aspen Mountain, 601 East Dean Street, Aspen, CO 81611:  On and preceding 11/18/2016, the employer did not ensure that the point of operation of a split-sheave installer was guarded.  The split sheave installer rotated sheaves, and these sheaves contained holes in the sheave hubs.  The rotating action created a pinch point hazard for the operators placing their hands near these rotating holes.  No physical barriers or other means of guarding was in place to prevent operators from placing their hands near the rotating sheaves.  This condition exposed operators to pinch-point and amputation hazards.

29 CFR 1910.1200(f)(6)(i): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the information required by 29 CFR 1910.1200(f)(1)(i) through 29 CFR 1910.1200(f)(1)(v):  (a) Aspen Skiing Company at Aspen Mountain, 601 East Dean Street, Aspen, CO 81611:  On and preceding 11/18/2016, the employer did not ensure that containers of Zep Brake Wash were properly labeled, tagged, or otherwise marked to identify the hazardous chemical contained therein.  This condition exposed employees to physical and health hazards associated with the use of this chemical.

29 CFR 1910.1200(h)(1): The employer did not provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new physical or health hazard the employees have not previously been trained about is introduced into their work area:  (a) Aspen Skiing Company at Aspen Mountain, 601 East Dean Street, Aspen, CO 81611:  On and preceding 11/18/2016,  the employer did not provide employees with effective information and training on hazardous chemicals in their work area, including, but not limited to:  1) Zep Brake Wash, containing Naptha, propan-2-ol, heptane, and methanol; and 2)  Magnaflux 14a containing 1-Napthalenecarboxaldehyde.  Employees worked with these chemicals on a regular basis while performing maintenance.  This condition exposed employees to physical and chemical hazards associated with the use of these chemicals.
