29 CFR 1910.147(c)(7)(i)(A): Each authorized employee did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control:    a) Throughout Facility - On or about November 21, 2016 and times prior to, authorized employees performing servicing and maintenance activities on equipment including but not limited to, the jot machines; saw rooms; tilt rolling conveyors; side cut saws; calibration machines; auto loaders; and polish machines, which had multiple energy sources including pneumatic, electric, and hydraulic did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy associated with each type of equipment, and how to isolate and control the different energy sources, thus exposing employees to caught-in, stuck-by, and crushed-by hazards.
