On or about 11/16/2016, employees engaged in HVAC demolition operations located at 1220 North Price, St Louis, MO, were exposed to a 23' 8" fall hazard from eave to ground.

Each employee on a walking/working surface having an unprotected side or edge which was 6 or more feet (1.8m) or more above a lower level was not-protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On about 11/16/2016, at the jobsite at 1220 North Price, St Louis, MO, employees engaged in HVAC demolition operations were not provided conventional fall protection. The employees were exposed to an approximately 23' 8" fall to the ground below.

29 CFR 1926.503(a)(1): The employer does not ensure that a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards. On or about 11/16/2016, at the jobsite at 1220 North Price, St Louis, MO, an employee engaged in HVAC demolition, fell approximately 23' 8" off of a roof. The employer failed to provide the employee with training on proper transfer of equipment and materials from roof to ground level.
