29 CFR 1926.405(g)(2)(v): Flexible cords and cables were not protected by bushings or fittings where passing through holes in covers, outlet boxes, or similar enclosures; a) Two extension cord outlet boxes had loose fittings with wires exposed to damage where they enter the metal box. One box had burn marks from a wire short-out. Location: 125 E 70th Street, New York, N.Y. On or about 11/16/2016 NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM IN ACCORDANCE WITH 29 CFR 1903.19.
