29 CFR 1910.106(e)(2)(iv)(c): Category 1 or 2 flammable liquids, or Category 3 flammable liquids with a flashpoint below 100 °F (37.8 °C), were used where there were open flames or other sources of ignition within the possible path of vapor travel. CHEM MIX 2 HOLDING AREA: On or about November 12, 2016, two employees were using ethyl acetate, a Category 2 flammable liquid, to clean hardened resin off the floor. The gas-fired boiler started and the ethyl acetate vapors were ignited resulting in a flashback.

29 CFR 1910.125(e)(1)(ii): Flames, spark-producing devices, or other surfaces that were hot enough to ignite vapors were not controlled in each vapor area and any adjacent area: COATING DEPARTMENT: On or about and at times prior to January 4, 2017, the tools used by employees when changing over the wet end of Coater AP-2 were not of the non-sparking type. Flammable liquids including, but not limited to, ethyl acetate, a Category 2 flammable liquid with a flash point of 26.6 deg F, were used in the coating and/or cleaning process.
