29 CFR 1926.451(a)(1): Each scaffold and/or scaffold component was not capable of supporting, without failure, its own weight and at least 4 times the maximum intended load applied or transmitted to it: (a) On or about 11/23/2016 - 7151 Jefferson Metro Parkway, McCalla, AL, employee(s) installing an elevator shaft were working from the third level of a scaffold with weathered and damaged boards when several planks broke exposing employee(s) to falls of approximately 23 feet.
