29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms shall be kept clean and orderly and in a sanitary condition:   Production Area: The employer is exposing employees to hazardous conditions where workplace floor is not kept clean, orderly, and in sanitary condition.  On 11/30/2016 the workplace floor was littered with refuse and loose concrete particles which created tripping hazards.

On 11/30/2016 forklifts (Toyota and Hyster) with defects such as, but not limited to, badly damaged tires, and missing slip resistant covers on brake pedals were not removed from service.

On 11/30/2016 and 12/07/2016 daily safety examinations were not conducted to identify defects such as, but not limited to, damaged tires, damaged signal tights, absence of slip resistant covers on brake pedals, and non-functioning horns.

On 11/30/2016 and again on 12/07/2016 employees were observed using a 2 ton overhead crane with damaged safety clasp on the hook.

A quadruplet receptacle box from which a flexible cord set was energized had a broken ground pin lodged in the upper right receptacle and a damaged lower left receptacle.

29 CFR 1910.334(a)(2)(ii): Where there was a defect or evidence of damage that could expose an employee to injury, the defective or damaged item was not removed from service until the repairs and tests necessary to render the electric equipment safe have been made:   Production Shop: The employer is exposing employees to hazardous conditions where damaged electric equipment was not removed from service.  On 11/30/2016, a yellow flexible cord set (extension cord) with no discernible markings was used by employees.  The flexible cord set had damaged insulation jackets at several spots and damaged internal wiring at another spot.
