29 CFR 1910.22(d)(1): On every building or other structure, or part thereof, used for mercantile, business, industrial or storage purposes, the loads approved by the building official were not marked on plates of approved design and securely affixed in a conspicuous place in each space to which they relate.  a) Location: Machine Shop: The load rating was not marked on the mezzanine level above the Supervisor's office and locker room. A break room and offices were located on the upper level. This exposes employees to fall hazards and/or struck-by hazards if the floor is overloaded.  b) Location: Sheet Metal Shop: The load rating was not marked on the mezzanine level above the tool room. Offices are located on the upper level. This exposes employees to fall hazards and/or struck-by hazards if the floor is overloaded.

29 CFR 1910.37(b)(4): Signs were not posted along the exit access indicating the direction of travel to the nearest exit and exit discharge when the direction of travel to the exit or exit discharge was not immediately apparent:  Location: Composite Work Area / Carpenter Shop The doorway between the Composite Work Area and the Carpenter Shop, was not marked with signage indicating the where employees would exit the work area, exposing employees to fire hazards.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  Location: Sheet Metal Shop: A periodic inspection of energy control procedures was not conducted annually to ensure employees maintained skill proficiency during maintenance and servicing activities. Employees were exposed to amputation hazards when not de-energizing equipment during die changes in the press brake.

Employees were exposed to amputation hazards due to risk of unexpected energization of equipment.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) Location: Machine Shop: The Bridgeport Series II vertical mill (#02914) did not have guarding affixed to prevent entanglement hazards from the cutting tool and/or struck-by hazards from flying chips.   b) Location: Machine Shop: The Bridgeport Series II vertical mill (#02935) did not have guarding affixed to prevent entanglement hazards from the cutting tool and/or struck-by hazards from flying chips.   c) Location: Machine Shop: The Lodge & Shipley power turn lathe did not have guarding affixed to prevent entanglement hazards from the rotating chuck.  d) Location: Sheet Metal Shop: Guarding on the Betenbender Mfg. Inc. shear was not adjusted properly and allowed for a 1-1/16 inch gap between the stock thickness and barrier guard, exposing employees to amputation hazards.

Employees were exposed to struck-by hazards.   b) Location: Machine Shop: The LARS Machine Inc. bench grinder had a tongue guard that was 7/8 inches away from the abrasive wheel. Employees were exposed to struck-by hazards.
