29 CFR 1910.36(d)(1): Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge: (a) The emergency exit door near the southeast corner of the building had a secondary dead bolt style lock installed and in a locked position. This would not allow an immediate exit from the facility. (b) The emergency exit door on the west side of the building near the emergency eye wash station had a secondary deadbolt style lock. This would not allow an immediate exit from the facility.

29 CFR 1910.37(a)(3): Exit route(s) were not kept free and unobstructed: (a) The emergency exit route and discharge door in the northeast corner of the facility was blocked by a large palletized filtration unit, two palletized 'A' frame ladders and several five gallon buckets of chemicals. (b) The east exit discharge route in the northeast corner of the facility was blocked by pallets and other materials stored in the exit discharge route.

29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer: (a) Employees in the warehouse were provided and occasionally used disposable dust masks during house keeping and inventory tasks to protect themselves from exposure to nuisance dust and upper respiratory tract irritation. Employees were not trained on the information of Appendix D of the Respiratory Protection Standard (29CFR1910.134) to inform employees of the usage and limitations of disposable dust mask respirators.
