A safe distance shall be maintained approximately three truck lengths from the truck ahead, and the truck shall be kept under control at all times. a. On or about November 21, 2016 speed control and traffic measures to control forklift traffic in high traffic area of the warehouse were not enforced exposing employees to struck by hazards.

29 CFR 1910.178(n)(8): Under all travel conditions the truck shall be operated at a speed that will permit it to be brought to a stop in a safe manner. a. On or about November 17, 2016 one employee was operating and Crown brand, Stand up Forklift and did not operate at a speed that will permit it to be brought to a stop in a safe manner, causing her to collide with another forklift resulting in permanent physical injuries to her left arm.
