The maximum permissible spans for 2- X 9-inch or wider planks are shown in the following table: a) Jobsite, 7710 Hamilton Avenue, Pittsburgh, PA: On or about November 29, 2016, two employees were working from a Safway scaffold that did not have any scaffold planks on the working level and were exposed to a fall hazard of approximately 15 feet to the ground.
