29 CFR 1910.145(c)(2)(i): Caution signs were not used to warn against potential hazards or to caution against unsafe practices:    a) 1605 Lancaster Ave -  The employer did not ensure that both of his Metered Concrete Trucks had caution signs warning employees of potential hazards including but not limited to; moving parts, do not put hands into or under running machinery or pinch points,  thereby exposing employees to caught in, crushed by or amputation hazards on or before November 14, 2016.     Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) 1605 Lancaster Ave, Columbia, Pa - An employee suffered a fractured arm while attempting to clean a unguarded 10 inch wide 9 foot long concrete auger when the employees arm become entangled in the auger causing broken bones, muscle and tendon damage on or before November 14, 2016.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours:    a) 1605 Lancaster Ave, Columbia, Pa - An employee suffered a fractured arm while trying to clean a 9 foot concrete auger operating at its lowest rpm. The employee was hospitalized on November 21, 2016. The employer did not report the in-patient hospitalization to OSHA.     No abatement certification or documentation required.
