29 CFR 1904.29(a): A log of all Work-Related Injuries and Illnesses (OSHA Form 300), and/or the Summary of Work-Related Injuries and Illnesses, (OSHA Form 300A) and/or the Injury and Illness Incident Report (OSHA Form 301) or equivalent forms were not kept by the establishment: (a) A Log of all Work-Related Injuries and Illnesses (OSHA Form 300) for calendar year 2015 was not being kept by the employer. Notice #1, Item #001, 29 CFR 1904.29(a) as required by 29 CFR 1960.8(b): The employer failed to ensure an OSHA 300 Log was being maintained. The employer was cited for a violation of the same standard on 10/22/14, Inspection #997492, 11/17/14 of final order/settlement, and was abated by creating the log.
