29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  (a). Worksite located at 1709 Tudor Lane, Liberty, Missouri- Employees working on the ground cleaning up and packing materials up on the roof were exposed to overhead hazards and were not provided any means of head protection.

Worksite located at 1709 Tudor Road, Liberty, Missouri- Employees were observed using nail guns and staple guns without the use of any means of eye protection.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  (a). Worksite located at 1709 Tudor Lane, Liberty, Missouri- Employees were exposed to fall hazards while working on a steep roof without the use of any  means of fall protection.

Worksite located at 1709 Tudor Lane, Liberty, Missouri- Employees were exposed to fall hazards in that the ladder being used to access the upper landing surface was not extended at least 3 feet above the upper landing surface.
