29 CFR 1926.652(a)(1): Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section except when: On or about November 30, 2016, during the installation of a sewer line at approximately 18627 Shurmer Road, Strongsville, Ohio, the employer did not ensure two employees working inside a trench, with a varying depth of approximately 65 inches to 80 inches, with near 90 degree vertical walls, were protected from cave-ins by an adequately designed protection system. The soil was classified as; Cohesive Soil, type B .
