29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent. On or about November 30, 2016, the employer did not record a workplace injury on the OSHA 300 form or equivalent in calendar year 2016. On November 16, 2016, a recordable injury occurred when an employee stepped on a piece of wood causing the employee to fall and suffer a sprained left ankle.
