29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: a. Employees were exposed to hazard associated with a floor opening, while performing frame work on the second level of a multi-story residential structure. On or about, 11/25/2016, an employee fell through the 10.5-ft to the ground floor opening and sustained multiple body lacerations when he landed on the ground. No fall protection had been provided, on or about, 11/25/2016.
