29 CFR 1926.102(a)(1): 29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:   a. On or about November 30, 2016, two employees using Hitachi NR83A2 pneumatic nail guns were not wearing safety glasses, exposing themselves to being hit by nails and flying debris.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a. On or about November 30, 2016, on a 6:12 pitched structure, two employees performing framing operations were not protected from a 8-foot 6-inch fall hazard by use of a guardrail system, safety net system, personal fall arrest system or any alternative fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a. On or about November 30, 2016, two employees used a 10-foot little john ladder to access the top of a structure improperly, in that the ladder did not extend 3 feet over the eave nor was secured from slippage, exposing the employee to a fall hazard.
