29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  Through out the shop;  On or about 11/30/16, lock out/tag out procedures were not developed to control the hazardous energies on each electron beam welding tool.  Employees that work on the electron beam welders need to control the following energies that can be potentially hazardous: electrical, which also controls the ionizing radiation hazard; vacuum pumps; cooling water.  Each machine needs for these energies to be controlled in different locations and sometimes by different means.

29 CFR 1910.1096(d)(1): The employer did not make such surveys as necessary to evaluate the radiation hazards incident to the production, use, release, disposal or presence of radioactive materials or other sources of radiation under a specific set of conditions:  Through out the shop;  On or about 11/30/16, radiation surveys conducted around electron beam welders did not indicate the location of measurements relative to the source.  The survey did not include measurements for multiple locations around the source to ensure that there were not leaks in the shielding.

The digital readout on the front of the box indicated that the box had power to it at the time of the inspection.
