29 CFR 1910.119(l)(4): Process safety information required by paragraph (d) of this section was not updated when a change covered by this paragraph resulted in a change in the process safety information.  On or about November 30, 2016, employees were exposed to hazards associated with ethanol production and the process safety information was not updated and reflective of the current operating process after implementing a management of change to eliminate ammonia from the process, including but not limited to, the Process Safety Management program overview and maximum intended quantity documentation.
