29 CFR 1910.179(j)(3): Complete inspections of the crane was not being performed at intervals as generally defined in paragraph (j)(1)(ii)(b) of this section: On or about November 29, 2016, and at times prior thereto, at this automotive engine repair shop production area, where employees hoisted heavy equipment and engines with overhead cranes, the employer did not perform any complete periodic inspections for the past 12 months for any defects of his overhead cranes components or functional operating mechanisms such as, but not limited to: a. Overhead crane (Jet 1000, 1 Ton); b. Overhead crane (Wright-Way, 1 Ton); c. Overhead crane (Hitachi, 2 Tons); d. Overhead crane (Yale, 1 Tons); and, e. Overhead crane (Duff-Norton, 1 Ton).

29 CFR 1910.1025(h)(1): All surfaces were not maintained as free as practicable of accumulations of lead: On or about November 29, 2016, in the motor machine shop and receiving warehouse work area, the employees worked in the loading and unloading the freight trailers with metal finished products and work surfaces were not regularly cleaned to prevent avoidable lead exposure, such as those potentially caused by re-entrained lead dust. Monitoring was conducted to ascertain lead presence and exposure. On November 29, 2016 the wipes samples were taken to ascertain lead exposure from different work surfaces, such as, but not limited to lunchrooms, shop equipment and employees work stations and desks: a) WIPE #1 162.85 ug/ft2 (Lunch Room; top surface of lunch wood table; lower right side); b) WIPE #2 56.00 ug/ft2 (Lunch Room; top surface of a black microwave); c) WIPE #3 131.825 ug/ft2 (Lunch Room; top surface of lunch wood table; upper left side); d) WIPE #4 61.59 ug/ft2 (Lunch Room; top surface of a white microwave); e) WIPE #5 41.34 ug/ft2 (Crankshaft department; top surface of operator working table/station); f) WIPE #6 156.62 ug/ft2 (Crankshaft department; top surface of operator working shelf); g) WIPE #7 21627.12 ug/ft2 (Connecting rods department; top surface of operator working table/station); h) WIPE #8 595.6748 ug/ft2 (Engine department; top surface of operator working table/station); and, i) WIPE #9 1768.816 ug/ft2 (Crankshaft department; top surface of operator right steel toe shoe).
