29 CFR 1926.652(a)(1): Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section.    a. Located near 5595 Bowen Road in Canal Winchester, Ohio: On or about December 1, 2016, the employer did not ensure that a protective system was in place to protect an employee from cave in hazards while grading gravel and installing sections of storm sewer pipe in an approximately six foot deep trench.
