29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person (i.e., a person who is capable of identifying existing and predictable hazards in the surroundings or working conditions which are unsanitary, hazardous, or dangerous to employees, and who has authorization to take prompt corrective measures to eliminate them. JOB SITE: 3700 Forest Park Avenue, Saint Louis, MO 63108, on or about 12/1/2016, the employer failed to provide for frequent and regular inspections of the jobsite by a competent person, to help identify existing and predictable hazards while workers were on the roof.

29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface were not protected from tripping in or stepping into or through holes: JOB SITE: 3700 Forest Park Avenue, Saint Louis, MO 63108, on or about 12/01/2016, a worker on the roof was not protected from stepping through holes, while inspecting and identifying soft roof concrete panels. The worker fell approximately 19' 7" to the concrete floor below.
