29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: The employer is failing to protect employees from corrosive materials being flushed from eyes with a blocked eye wash station. This was most recently documented on December 2, 2016 in the northwest corner of the warehouse at Apogee Trucking Logistics LLC in Kansas City, Missouri. The eye wash station located in the northeast corner of the warehouse was blocked by bicycles and a forklift.

29 CFR 1910.212(a)(5): Fan blade guard was not provided where the periphery of the blades was less than seven feet from the floor or working level: The employer is failing to protect employees from the periphery of the blades of a floor fan. This was most recently documented in the northwest corner of the warehouse at Apogee Trucking Logistics LLC in Kansas City, Missouri. The white floor fan located in the northwest corner of the warehouse had an opening larger than one inch when the cover of the fan was missing.
