29 CFR 1910.106(b)(6): In locations where flammable vapors may be present, precautions were not taken to prevent ignition by eliminating or controlling sources of ignition.  Sources of ingition may include open flames, lightning, smoking, hot surfaces, frictional heat, sparks (static, electrical, and mechanical), spontaneous ignition, chemical and physical-chemical reactions, and radiant heat:    a) On or about November 23, 2016, at the cave yard wash bay area, employees were exposed to serious fire and explosion hazards in that one or more propane torches were used on and around truck #932 while the tank of the truck was loaded with flammable MC800. An employee suffered first and second degree burns to his head, neck, arm, and hand when vapor escaped the tank and ignited.    b)  On or about November 23, 2016 and dates prior, at the cave yard wash bay area, employees were exposed to serious fire and explosion hazards in that one or more propane torches were used on and around truck #932 while the truck body and tank were sprayed with "Milk Solution" as part of the truck cleaning process.

29 CFR 1910.1200(e)(1): The employer shall develop, implement, and/or maintain at the workplace a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, safety data sheets, and employee information and training will be met, and which includes the following:    (ii) The methods the employer will use to inform employees of the hazards of non-routing task (for example, the cleaning of reactor vessels), and the hazards associated with chemicals contained in unlabeled pipes in their work areas.    On and before November 23, 2016, at the cave yard wash bay area, employees were exposed to serious fire, explosion, and chemical hazards, in that the employer had not trained its employees on the hazards of the non-routing task of cleaning the vehicle with specifically , the cleaning of distributor truck #932 using chemicals with flammable vapors such as MC-800 in proximity to propane torches.
