29 CFR 1926.300(b)(2): Moving part(s) of equipment exposed to contact by employees or which otherwise created a hazard, were not guarded: a) Square One located at 6050 Roswell Road, Sandy Springs, GA 30328, Corner of Roswell Rd. & Sandy Springs Place: On or about November 25, 2016, and at times prior, the employer exposed employees to amputation hazards from the capstan winch used on site to hoist stucco materials to the upper levels, in that the cover of the Honda starter recoil flywheel was not guarded which exposed operators to hazards.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: (a) Square One Apartments, Parapet wall: On or about November 25, 2016,and at times prior, the employer exposed employees to fall hazards of 75 feet from the lower ground level, in that a fall protection system was not provided to employees installing stucco while working from the top of the parapet wall.
