29 CFR 1926.1053(b)(4): Ladders were used for purposes other for which they were designed. A. Coyle Mechanical, Inc. worksite located at 545 N. Huron St., Toledo, Ohio: On or about November 30, 2016, the employer failed to ensure a 8 ft A-framed stepladder was opened during use in a trench . The ladder was closed and leaning against the sidewall during use. An employee was using the ladder in the fore mentioned condition while entering and exiting the trench.
