29 CFR 1915.73(d): When employees were exposed to unguarded edges of decks, platforms, flats, or similar flat surfaces, more than 5 feet above a solid surface, the edges were not guarded by adequate guardrails meeting the requirements of 29 CFR 1915.71(j)(1) and (2).  a)    USS Oak Hill, 08 level, Aft: On or about 11/14/2016, an employee worked on top of the Sea Whiz Mount near the edge to weld in a structural plate and was not protected by guardrails or a personal fall arrest system, as a result was exposed to an approximate 8 and 20 feet fall.   No abatement certification or documentation required.

29 CFR 1910.1200(h)(3)(iv): Employee training did not include the details of the hazardous communication program developed by the employer, including an explanation of labels received on shipped containers and the work place labeling system used by their employer, and the safety data sheet, including the order of information and how the employees can obtain and use the appropriate hazard information:  a)    USS Oak Hill, Sea Whiz Mount: On or about 11/14/2016, an employee was not trained regarding the new label elements and safety data format for welding wire used to weld inserts.   Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1915.502(a): A written fire safety plan was not been developed and implemented by the employer that covered all of the actions necessary to ensure the employees safety in the event of a fire.  a)    USS Oak Hill, Sea Whiz Mount: On or about 11/14/2016, a written fire safety plan was not developed and implemented to ensure employees safety in the event of a fire.  Note: The employer must include the following information in the fire safety plan:  1.    Identification of the significant fire hazards; 2.    procedures for recognizing and reporting unsafe conditions; 3.    Alarm procedures; 4.    Procedures for notifying employees of a fire emergency; 5.    Procedures for notifying fire response organizations of a fire emergency; 6.    Procedures for evacuation; 7.    Procedures to account for all employees after an evacuation; and 8.    Names, job titles, or departments for individuals who can be contacted for further information about the plan.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
