29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours: A.) On or about Monday, November 7, 2016 at Jamaica Hospital, 89-00 Van Wyck Expressway, Jamaica, NY 11418 The employer failed to report to OSHA the in-patient hospitalization of an employee within twenty-four hours that occurred on November 7, 2016. The employee in-patient hospitalization was not reported to OSHA until November 18, 2016. ABATEMENT CERTIFICATION IS NOT REQUIRED
