29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    (a)  Packaging & Pads R Us, LLC on or about November 16, 2016, at the address of 12406 US Hwy 250 N Milan, OH: The employer did not ensure ensure the incoming nip point between the scoring bar and drive roller contained on the Line #2 paper forming machine was adequately guarded. Operating employees were exposed to amputations hazards at the aforementioned unguarded nip point (point of operation) as operators clear miss fed waste product.   When reaching into the machine to clear waste material during a production run, the operator's hand was within the proximity of the machine's inadequately guarded scoring bar.     (b)  Packaging & Pads R Us, LLC on or about November 16, 2016, at the address of 12406 US Hwy 250 N Milan, OH: The employer did not ensure ensure the incoming nip point between the side slitting wheels and drive roller contained on the Line #2 paper forming machine were adequately guarded. Operating employees were exposed to amputations hazards at the aforementioned unguarded nip point (point of operation) as operators clear miss fed waste product.   When reaching into the machine to clear waste material during a production run, the operator's hand was within the proximity of the machine's inadequately guarded side slitting wheels.
