29 CFR 1926.502(b)(1): The employer did not ensure that the top edge height of top rails, or equivalent guardrail system members, were 42 inches (1.1 m) plus or minus 3 inches (8 cm) above the walking/working level, or when conditions warranted, that the height of the top edge exceeded the 45-inch height, provided the guardrail system met all other criteria of this paragraph: Instances including but not limited to the following: a) Job site, 2080 Mackenzie Way, Cranberry, PA: On or about December 2, 2016, during roofing work near the ladder entry area, the employer did not ensure that guardrail systems used for fall protection purposes were compliant. The lower parapet wall measured approximately 22 inches from the roof, the guardrail mid rail cable measured approximately 36 inches from the roof and the guardrail top rail cable measured approximately 51 inches from the roof.

The wire rope cables installed for the guardrail system were not flagged at no more than 6 foot intervals with high visibility material.
