Throughout facility: On or about 1/3/17 the employer did not record the following workplace injuries or illnesses on the OSHA Form 300 or equivalent for the calendar year 2016 within seven calendar days:    a) CVC Thermoset Specialties 2980 Route 73 North Maple Shade, NJ 08052: On 12/17/16 an employee was exposed to epichlorohydrin while removing pipe that attached to reactor R-103. The employee suffered third degree burns to his foot and was hospitalized.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours:     a) CVC Thermoset Specialties 2980 Route 73 North Maple Shade, NJ 08052: On or about 12/17/2016, an employee received 3rd degree chemical burns to the top of his foot resulting in a hospitalization that occurred on 12/20/17. The hospitalization was reported to OSHA on 12/23/2016, not within the required 24 hours.
