29 CFR 1910.157(g)(2): The educational program to familiarize employees with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting was not provided to all employees upon initial employment, and at least annually thereafter: On or about December 6, 2016, and times prior thereto, the employees are required to use the fire extinguisher to put out incipient stage fires. There was no annual fire extinguisher training accomplished. Employees exposed to the hazard of being burned.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about December 6, 2016, and times prior thereto, at the Craftsman 15 inch drill press, the drill press was not guarded or shielded. The employees will drill various metal items. Employees were exposed to the hazard of being caught-in the drill press.

29 CFR 1910.212(a)(5): Fan blade guards, where the periphery of the blades was less than seven feet above the floor or working level, had openings larger than one half inch: On or about December 6, 2016, and times prior thereto, the Emerson electric fan was not adequately guarded. The fan was used for cooling and for the vapors produced from the G10 circuit boards. Employees were exposed to the hazard of being struck-by the rotating blades in the fan.

29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: On or about December 6, 2016, and times prior thereto, the Emerson electric fan was not grounded. The fan was being used for cooling and for vapors from G10 work on circuit boards. Employees were exposed to the hazard of an electrical shock from the ground pin missing.
