29 CFR 1926.405(b)(2): Covers and canopies. All pull boxes, junction boxes, and fittings shall be provided with covers. If metal covers are used, they shall be grounded. In energized installations each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear. a. On or about October 31, 2016 at the above referenced worksite, employees were engaged in demolition and / or Gut-ReHab activities. Employees were working in close proximity to an open panel. When tested, the panel was deemed active and live. The panel was listed and labeled as 480 volts. Employees were thereby exposed to electrical shock. b. On or about October 31, 2016 at the above referenced worksite, employees were engaged in demolition and / or Gut-ReHab activities. Employees were working in close proximity to an un-covered raceway. When tested, the raceway was deemed active and live. The raceway was not listed and / or labeled, but it was servicing a 120 outlet, as outlined in item c. Employees were thereby exposed to electrical shock. c. On or about October 31, 2016 at the above referenced worksite, employees were engaged in demolition and / or Gut-ReHab activities. Employees were working in close proximity to an open junction box. When tested, the junction box was deemed active and live. The junction box was in-line from the panel listed in item a and the outlet mentioned in item b, and was not otherwise listed and labeled as <120 volts. Employees were thereby exposed to electrical shock.
