Jarrell Contracting Company, Inc. was seriously injured when a tractor trailer rolled away from the Receiving Dock causing his forklift to fall to the ground level. This Receiving Dock is located at 4800 Park 370 Blvd., Hazelwood, MO. The vehicles brakes were not set nor the wheels chocked during the forklift activity of unloading the trailer.

29 CFR 1910.178(m)(7): Brakes shall be set and wheel blocks shall be in place to prevent movement of trucks, trailers, or railroad cars while loading or unloading. Fixed jacks may be necessary to support a semitrailer during loading or unloading when the trailer is not coupled to a tractor.
