On or about 11/28/2016, working sites - employees were required to break mulching tunnels using pitch folks inside the blower trucks were exposed to a 12 feet 2 inch fall hazard, in that, fall protection was not provided when climbing on top of the truck and mulch cargo box.

On or about 11/28/2016, working sites - employees required to break mulching tunnels inside the blower trucks were exposed to impalement and amputation hazard, in that, they are required to enter inside the mulching box while equipment such as a conveyor belt, augers and stir sticks were operating and the employer did not evaluate the space to determine as a permit-required confined spaces.
