Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly. On or about December 6, 2016, employees were using Harlo HC 5600 forklift Model # 85594, S/N 64043 with approximately 112 inch fork pockets to move equipment and supplies without knowing the true capacity of the forklift there by exposing employees to struck by or crushing hazards.

On or about November 28, 2016, Toledo Beach Marina employees used the 20 ton Kleeco Travel-Lift, Model #SP400H, S/N 0183ZH to move a 25 foot Catalina Sailboat out of the water and did not have rated load markings on each hoisting unit or load block exposing employees to struck against, struck by, caught between or under hazards. b. On or about December 6, 2016, Toledo Beach Marina employees used the 25 ton Marine Travelift, Model #25 AMG, S/N 9641066 to move a 41 foot Chris Craft boat out of the water and did not have rated load markings on each hoisting unit or load block exposing employees to struck against, struck by, caught between or under hazards. c. On or about November 28, 2016, Toledo Beach Marina employees used the 60 ton Marine Travelift, Model #60 AMO, S/N 1605979 to move a 45 foot wooden troller out of the water and did not have rated load markings on each hoisting unit or load block exposing employees to struck against, struck by, caught between or under hazards.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure: Toledo Beach Marina, the employer used flexible cable as a substitute for fixed wiring there by exposing employees to an electrical hazard. Specifically: a. On or about December 6, 2016, in the Rag Man shop, flexible cord was used to power a 4 way outlet which provided power to a light fixture on the exterior of the building above the entrance door and provided power to a surge protector used to power various items in the Rag Man shop. b. On or about December 6, 2016, in the Rag Man shop, flexible cord was used to power 30 amp circuit mounted on the wall which provided power to a recreation vehicle within the building.
