29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  (a) 941 Caspian Drive York, PA Residential home - Employee(s) on a residential roof were observed performing roofing work activity on a 7 in 12 pitch main residential roof 9 feet 61/2 inch high, without the benefit of a fall protection system, thereby exposing employees to a fall to the ground below, on or about December 7, 2016.      No abatement certification or documentation required.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a)  941 Caspian Drive York, PA  17404 Residential home - The employee was performing roofing work activity on a residential roof at height of approximately 9 feet 61/2 inch to approximately 15 feet where the employees were not protected by a fall protection system, and the employer did not provide fall protection training to the employees to recognize a fall hazard, thereby exposing employees to serious injury, on or about December 7, 2016.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:     (a)  941 Caspian Drive York, PA  Residential home - Employees were observed using an aluminum extension ladder that did not extend at least 3 feet above the landing surface to the front garage roof, thereby exposing employees to a fall hazard of approximately 9 feet 6 1/2 inch to the ground below, on or about December 7, 2016.   No abatement certification or documentation required.
