29 CFR 1910.119(d)(3)(ii): The employer did not document that equipment complies with recognized and generally accepted good engineering practices: a) Machine Room, on or about December 14, 2016 - The employer did not comply with recognized and generally accepted good engineering practices (RAGAGEP) in that the ammonia machinery room was not equipped with tight-fitting doors in the machinery room. ANSI/ASHRAE Standard 15-2013, Section 8.12, Paragraph B, is an applicable RAGAGEP that describes the requirements. b) Piping System, on or about December 14, 2016 - The employer did not comply with recognized and generally accepted good engineering practices (RAGAGEP) in that the ammonia piping system on the roof was not labeled. ASME A13.1- 2015 and IIAR Bulletin #114, are applicable RAGAGEP that describes the requirements.

Excessive ice build-up was observed on the piping system in the facility.
