29 CFR 1926.62(d)(1)(i): Each employer who has a workplace or operation covered by this standard shall initially determine if any employee may be exposed to lead at or above the action level. Quasius Construction Inc. project site located at 133 E. Mill St. Plymouth, WI: On and before December 7, 2016, employees were exposed to demolition materials containing lead based paint while renovating a commercial building that was more than 100 years old when they had not been informed of the presence of, or potential of toxic materials in the workplace.

Mill St. Plymouth, WI: On December 7, 2016, a Green bull 24' fiberglass extension ladder was positioned for use when the bottom rung was partially broken out of the side rail.
