29 CFR 1910.134(c)(2): The employer did not develop and implement a respiratory protection program in accordance with sections i and ii of this paragraph (c)(2) for employees who chose to wear respirators where respirator use was not required:    a.  On or about December 5, 2016, in the facility, employees were provided tight fitting negative pressure respiratory protection.  Employees were exposed to respiratory hazards when the employer failed to establish and implement a written respiratory protection program.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    a.  On or about December 5, 2016, in the facility, employees were required to perform maintenance activities including but not limited to changing filters.  The employer did not have a written lock out/tag out program which included energy control procedures, employee training and periodic inspections of the program.  The employees were thereby exposed to the hazards associated with the unexpected energizing or start up of machines and equipment.

29 CFR 1910.303(b)(1): Examination. Electric equipment was not free from recognized hazards that are likely to cause death or serious physical harm to employees. Safety of equipment shall be determined using the following considerations:    a.  On or about December 5, 2016, on the forth floor, an electrical cord used to charge a powered industrial vehicle had damaged insulation and was without a ground fault circuit interrupter, thereby exposing employees to electrical hazards.
